Geriatric
Care Management Assessment and Planning Consultation:

During
this meeting, information is obtained regarding health concerns,
functional and cognitive status, living situation, social history,
current support systems, emotional needs and financial/legal resources
of the elder client. Often a Geriatric Care Manager conducts a home
assessment as well to identify the areas with which the client may
need assistance. Services are recommended to assist the individual
in maintaining maximum independence and to promote health, wellness,
and safety. The Geriatric Care Manager also may make recommendations
for care and referrals to appropriate community resources.
This assessment may serve as a one time planning consultation session
or may serve as the prelude for ongoing monitoring services. |
